One of Australia’s leading health services set out to reimagine how they coordinate care with patient at the centre of everything they do.. As part of a broader public healthcare system, the service works closely with multi disciplined healthcare teams to improve healthcare, coordination, and the overall patient experience.
As part of its digital transformation efforts, the service led the development of a secure online portal to help patients, their authorised families and care givers to be more involved and informed of appointments, treatment and referrals.
The challenge
The vision was clear: create a mobile-friendly portal that gives patients and families more visibility and control over their care journey. But early technical hurdles needed to be resolved to keep the program on track.
We were initially brought in for a short engagement to troubleshoot integration issues between the new portal and the existing electronic medical record (eMR) system, and to identify a path forward for automating master data management. What started as a narrow scope quickly grew into a broader technical partnership as the full complexity of the solution came into view.
Our approach
The portal has been supporting patients and authorised family members for some time now, making it easier to coordinate clinical care — anytime, anywhere.
Through the app or web interface, patients can:
- View upcoming outpatient and community appointments
- Request changes or reschedule if needed
- Update their contact details
- Receive directions and appointment reminders
- Communicate with clinic staff in real time
- Access personalised health information and educational materials
The initial rollout marked a major milestone — the first of its kind in the state — and has since expanded to other health services.
For families, it’s a smoother, more transparent way to manage care. For clinical teams, it’s a chance to reduce paperwork and focus more time where it matters most.
